This position is based onsite at our well-equipped rehabilitation facility in Fords, New Jersey. Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients with various physical challenges to create tailored treatment plans.
• Implement therapeutic interventions that may include exercises, manual therapy techniques, and modalities, ensuring adherence to individualized care plans.
• Educate patients and their families on recovery processes, injury prevention, and lifestyle modifications to facilitate long-term health improvements.
• Collaborate with an interdisciplinary team of healthcare professionals, including physicians and nurses, to ensure a seamless continuum of care.
• Maintain detailed documentation of patient progress, treatment effectiveness, and communication with other healthcare providers.
• Participate in community outreach initiatives and contribute to program development and quality improvement efforts.

Patient Population / Client Focus

As a Physical Therapist in our facility, you will work with a diverse patient demographic, including individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain issues. Your focus will be on empowering patients through functional rehabilitation and education to improve their movement, balance, and overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a higher degree (Master's or Doctorate) is preferred.
• A valid state license to practice Physical Therapy in New Jersey.
• Strong verbal and written communication skills to effectively interact with patients and healthcare teams.
• Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.

Preferred Experience
