Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychosocial needs to create personalized treatment plans.

• Provide case management support, including coordinating short-term counseling and crisis intervention services tailored to individual circumstances.

• Guide clients through the process of obtaining and completing necessary paperwork for various entitlements, insurance benefits, and other resources that can alleviate their reliance on emergency assistance.

• Assist clients in accessing pharmaceutical prescription assistance programs provided by community organizations and drug manufacturers.

• Actively advocate for patients within community agencies to ensure they receive the necessary services and support.

• Keep meticulous and updated patient records, compiling reports and documentation to track patient progress and treatment efficacy.

• Organize transportation assistance for clients when needed to facilitate access to services.
\n
• Participate in clinical meetings and case conferences to continuously align care strategies with team members.

Qualifications\n

Compensation and Benefits\n
