This remote position allows for flexibility while also providing access to our outpatient office for patient sessions and team collaborations. Key Responsibilities\n

• Conduct thorough mental health assessments, understanding how both psychological and medical conditions intertwine to affect patients’ mental well-being.

• Facilitate individual and group therapy sessions employing evidence-based therapeutic modalities customized to patients' specific circumstances.

Patient Population / Client Focus\n

You will primarily work with patients experiencing co-occurring mental health and medical conditions, such as chronic pain, autoimmune disorders, and substance abuse issues. Your ability to empathize and understand the complexities of these diagnoses will be essential in providing high-quality care and support.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LCPC, or LMFT in the state of Illinois.
\n
• Proficiency in addressing the mental health needs of patients with co-occurring medical conditions, along with practical experience in integrated care settings.

• Excellent interpersonal and communication skills that foster positive relationships with patients and team members alike.

• A strong understanding of and adherence to ethical guidelines, ensuring patient confidentiality and professional boundaries.

• Familiarity with electronic health record (EHR) systems and relevant health informatics tools.

Compensation and Benefits\n
