Key Responsibilities\n

As a Licensed Professional Counselor at Congruent Health Center, you will take on a variety of responsibilities, including:

• Conducting comprehensive assessments to determine the needs of each patient and tailoring personalized care plans.

• Communicating effectively with supervisors, family members, and other care team members regarding each patient's progress and any necessary adjustments to the treatment plan.
\n
• Preparing and managing an organized schedule of appointments, home visits, and consultations to ensure continuity of care and adherence to the established program plan.
\n
• Conducting home visits to gather necessary information, including chart reviews and staff interviews, while also applying data analysis to enhance service delivery.
\n
• Guaranteeing effective implementation of treatment plans and actively participating in Interdisciplinary Team Meetings to discuss patient progress and strategies.
\n
• Documenting progress through timely completion of monthly and quarterly notes to create a clear history of each patient's journey.
\n
• Providing training sessions to ensure competency among staff and other team members in the implementation of program plans and documentation procedures.
\n
• Offering follow-up interviews and support visits to ensure ongoing care adjustments based on patient feedback and changing circumstances.
\n
• Maintaining accurate records to facilitate easy access to patient information for involved parties.
\n
• Engaging with the broader mental health community to foster collaborations that align with the center's goals for community development.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population, including both adults and youth, who are confronting a range of mental health issues. This may include anxiety, depression, trauma, substance misuse, and family and relationship challenges. Your expertise will be instrumental in helping patients understand their experiences and develop strategies for healing and growth. Specialization in family therapy, couples therapy, substance use disorders, or trauma is highly valued, allowing for a more tailored approach to treatment.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Texas.

• A minimum of 2 years of counseling experience, preferably in clinical settings.

• Strong understanding of legal and ethical principles regarding mental health counseling.

• Proven interpersonal and communication skills, with an ability to build rapport with clients from diverse backgrounds.

• Demonstrated capacity to collaborate effectively within a multi-disciplinary healthcare team.

• Proficiency in electronic medical record (EMR) systems and relevant technologies is highly desirable.

Preferred Experience\n

Compensation and Benefits\n
