Key Responsibilities\n

As a Clinical Mental Health Counselor, you will be responsible for:

• Conducting thorough psychosocial assessments to diagnose and treat various mental, emotional, and behavioral disorders.

• Developing and modifying individualized care plans that adapt to changes in patient status to enhance treatment outcomes.

• Collaborating with multidisciplinary teams to create comprehensive discharge plans, ensuring patients receive necessary follow-up services and resources.

• Maintaining compliance with state and federal regulations, adhering to the Mental Health Practice Act, and upholding clinical facility standards.

• Facilitating social work consultations, conducting psychosocial assessments, and applying supportive interventions as needed.

• Engaging in effective communication with multiple patient care teams and external agencies to guarantee timely patient discharges.

• Acting as an advocate for patients and families and coordinating with care providers to address any barriers to discharge.

Patient Population / Client Focus\n

This role primarily focuses on a diverse adult population facing varying levels of mental health challenges, including anxiety, depression, and trauma-related issues. Qualifications\n

To excel in this role, candidates should possess the following qualifications:

• A Master's Degree in Social Work or a related field from an accredited institution, with verification required.

• Current State Licensure as an LCSW, LMFT, LMHC, or Psychologist.

Compensation and Benefits\n
