Key Responsibilities\n

• Provide empathetic mental health counseling and educational services to patients, focusing on coping strategies and emotional support tailored to their individual needs.

• Facilitate communication and collaboration within an interdisciplinary team, anticipating client needs and actively participating in transition conferences for at-risk patients.

• Monitor patient progress through regular documentation in electronic medical records, ensuring that treatment goals are achieved and that plans are adjusted as necessary in response to patient feedback.

• Assure compliance with reporting mandates regarding vulnerabilities or suspected abuse, advocating for patients' rights and well-being.

• Engage in proactive measures to prevent patient readmissions by utilizing evidence-based assessments and technology to identify individuals at risk.

• Support families by providing education on resources and self-care strategies to enhance their ability to contribute positively to the patient’s recovery journey.

• Regularly review governmental and private payer networks to aid in effective and appropriate patient transitions.

Qualifications\n

• A Master's or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited institution (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

Compensation and Benefits\n
