Key Responsibilities\n

• Conduct thorough assessments on an ongoing basis to establish the individual needs of each client and develop respective treatment plans.

• Collaboratively communicate with supervisors, direct care staff, family members, and relevant stakeholders to discuss individual progress and any necessary adjustments to care.
\n
• Provide a tentative schedule to supervisors detailing upcoming appointments, home visits, and observations aimed at monitoring client progress.
\n
• Coordinate and conduct home visits to ensure a comprehensive understanding of clients' environments, which includes chart reviews, interviewing staff, and data analysis.
\n
• Implement and oversee treatment plans to guarantee their efficacy and proper adherence by all involved parties.
\n
• Participate in Interdisciplinary Team Meetings and other essential meetings to discuss cases and share insights on treatment strategies.
\n
• Document monthly and quarterly progress notes illustrating client advancements and ongoing needs.
\n
• Organize and facilitate staff training sessions to ensure competence in implementing the program plan and proper documentation practices.
\n
• Conduct follow-up interviews and additional home visits to continually assess and support the evolving needs of clients.
\n
• Maintain detailed records, ensuring that crucial information is accessible to authorized team members and family when necessary.

• Act as a liaison between Mental Thrive and the broader mental health community to support developmental goals and initiatives.
\n
• Contribute to developing and refining training materials aimed at improving programs and outcomes for individuals served.

Patient Population / Client Focus\n

You will have the opportunity to work with diverse populations including adults and youth, addressing a wide array of mental health challenges such as anxiety, depression, trauma, and substance misuse. Your role is critical in delivering tailored counseling services that enhance the quality of life for our clients, helping them navigate through their struggles and fostering resilience.

Qualifications\n

• Possession of a Master's or Doctoral degree in Counseling, Psychology, or a closely related discipline.

• Active and unrestricted licensure as a Professional Counselor (LPC) in New Jersey.
\n
• A minimum of 2 years of clinical counseling experience, ideally in a direct healthcare setting.

• A solid understanding of legal and ethical standards related to mental health practice.
\n
• Exceptional interpersonal skills, capable of establishing rapport and effective relationships with a wide range of clients.

• Demonstrated ability to operate collaboratively as part of a healthcare team, enhancing patient care and outcomes.

• Familiarity and proficiency with Electronic Medical Record (EMR) software and related technologies.

• Annual salary range of USD 90,000 – 100,000, contingent upon experience and caseload management.
