Key Responsibilities\n

• Triage both urgent and routine referrals from medical providers, ensuring timely access to mental health services for patients.

• Collaborate closely with staff and medical providers to coordinate patient intake appointments and service delivery.

• Participate in care team huddles to discuss case progress, share insights, and implement treatment strategies.

• Develop comprehensive and client-centered treatment plans tailored to the specific needs of your assigned clients, integrating evidence-based practices.

• Engage in case management and advocacy efforts, including medication monitoring, emergency service coordination, insurance liaising, and collaboration with other agencies as necessary.

Qualifications\n
