Key Responsibilities\n

• Engage in ongoing communication with healthcare providers, including physicians, to report on patient progress, identify needs, and discuss any concerns to formulate effective care plans.

• Maintain precise and timely clinical documentation, including progress notes and treatment plans, in compliance with departmental protocols and regulatory standards.

• Provide immediate support and intervention for clients experiencing crises, using crisis intervention techniques tailored to the specific needs of hospice care.

• Regularly evaluate and document clients' progress toward their treatment goals, adjusting therapeutic approaches as necessary based on evidence-based practices.

• Collaborate effectively with family members and the care team to develop comprehensive discharge planning, ensuring a smooth transition and continuity of care.

Patient Population / Client Focus\n

This role primarily focuses on serving clients in hospice care, who may be dealing with terminal illnesses and significant life challenges. You will work closely with patients and their families, addressing emotional, psychological, and social issues that arise during the end-of-life process. Your therapeutic interventions will provide essential support, helping clients navigate their feelings and experiences, ultimately improving their quality of life.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

Compensation and Benefits\n
