Key Responsibilities\n

Your day-to-day responsibilities will encompass a broad range of tasks aimed at promoting client health and well-being. Key duties include:

• Developing and implementing individualized treatment plans based on assessment outcomes, and modifying these plans as necessary to reflect changes in client status.

• Collaborating with clients' families, healthcare providers, insurers, and community resources to ensure a cohesive approach to care.

• Identifying critical crisis situations and providing immediate support, resource referrals, and necessary interventions to address clients' urgent needs.

• Facilitating discharge planning in coordination with the multidisciplinary team, ensuring clients have access to additional services for ongoing support after leaving care.

• Adhering to all federal, state, and local clinical practice standards, as well as the guidelines established by the Mental Health Practice Act.

• Maintaining accurate client records and documentation in compliance with HIPAA regulations, ensuring the confidentiality and integrity of all client information.

Qualifications\n

Compensation and Benefits\n
