Key Responsibilities\n

• Conduct comprehensive assessments of patients' mental health to determine their needs and tailor appropriate treatment plans.

• Effectively communicate with supervisors, direct care staff, and involved family members about each patient’s progress and potential changes in treatment interventions.

• Develop and maintain a tentative schedule for appointments, ensuring thorough documentation of home visits and other observations critical to monitoring patient advancement.

• Coordinate in-home visits to conduct observations, complete chart reviews, and gather data through staff interviews to evaluate patient needs and program effectiveness.

• Ensure that all implementation of program plans is executed effectively, regularly revising strategies based on patient responses and progress.
\n
• Participate in Interdisciplinary Team Meetings and other collaborative sessions to discuss patient care and holistic wellness approaches.

• Prepare and maintain monthly and quarterly progress notes, documenting patient advancements and setbacks meticulously.
\n
• Organize training sessions aimed at ensuring that staff and team members are proficient in implementing program plans and maintaining accurate documentation.

• Provide continual support through follow-up interviews, home visits, and careful observations of patients, ensuring their ongoing needs are met.

• Act as a liaison between the organization and the broader mental health community, fostering partnerships that advance development goals and patient support systems.

Patient Population / Client Focus\n

You will serve a varied patient population that includes both adults and youth dealing with diverse mental health challenges. Your role as a counselor will involve the application of your skills in family therapy, couples therapy, trauma resolution, and substance misuse therapy to support individuals in overcoming their unique hurdles. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
