Work Environment

This position operates primarily in a home care environment, allowing our therapists to give hands-on treatment tailored to each client's unique circumstances. You will visit patients in their homes, providing the flexibility to adapt treatment plans based on their living conditions and personal needs. This setting fosters strong relationships with clients and their families, making a significant impact on their rehabilitation journey.

Key Responsibilities

• Direct physical therapy treatments to prevent disability, alleviate pain, improve, and restore muscle function, and maintain maximum performance based on individualized care plans.
• Supervise physical therapy assistants to ensure the accurate execution of the treatment plan.
• Evaluate clients' functional levels using diagnostic tests related to muscle, joint, nerve, and overall functionality.
• Assess home environments to provide recommendations that enhance patient independence and safety during rehabilitation.
• Administer prescribed physical therapy treatments, including exercises, manual therapy, and application of physical modalities such as traction and massage.
• Evaluate and adjust orthotic and prosthetic devices, recommending modifications as needed in consultation with treating physicians.
• Participate in interdisciplinary team meetings to develop, review, and update comprehensive care plans, ensuring coordinated patient care among various healthcare providers.
• Document patient progress through clinical records, including initial evaluations, care plans, and discharge summaries, ensuring compliance with regulatory standards.
• Guide clients and families on physical therapy techniques, device usage, and overall treatment strategies.
• Engage with case managers and physicians by reporting any patient condition changes and collaborating on care modifications.
• Complete functional assessments periodically to refine the care plan based on patients' evolving needs.

Patient Population / Client Focus

You will serve a diverse clientele, typically comprising elderly patients recovering from surgeries or chronic illnesses and individuals with mobility impairments. By fostering meaningful connections with each patient, you facilitate a supportive atmosphere that addresses both their physical needs and emotional concerns, contributing positively to their overall quality of life.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice physical therapy.
• A current CPR certification and a valid driver's license with automobile insurance.
• Strong interpersonal and communication skills, with a focus on patient engagement and collaboration within multidisciplinary teams.
• Commitment to upholding standards of clinical excellence, quality of care, and ethical practice.

Preferred Experience

A minimum of two years of clinical experience as a Physical Therapist, ideally within home care or acute care settings, is preferred. Compensation and Benefits

Symbria Rehabilitation Center offers a competitive salary ranging from USD 100,000 to USD 120,000 annually, commensurate with experience and credentials.
