Work Environment\n

This position allows for flexible remote work while primarily engaging with clients through a school-based setting. Our team collaborates closely with local educational institutions to provide necessary interventions and support systems for students and their families. Typical work hours are Monday to Friday, from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be necessary to meet client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, focusing on emotional, mental, and social factors affecting their daily lives.

• Design and implement individualized care plans in collaboration with clients and their families, ensuring alignment with specific goals and personal circumstances.

• Provide ongoing counseling services to seniors and their family members, helping them navigate significant life transitions, such as loss, retirement, or health challenges.

• Act as an advocate for clients’ rights, facilitating their access to essential community resources, services, and social engagement opportunities.

• Collaborate with interdisciplinary teams, including health professionals and caregivers, to coordinate care effectively and address multiple facets of client needs.

• Conduct regular check-ins to evaluate client progress, monitor living conditions, and identify any concerns that may impact safety and wellness.

• Facilitate open communication among family members, assisting in conflict resolution and education about available support systems.

• Document all assessments, care plans, and client interactions accurately to ensure continuity of care and compliance with ethical standards.

• Stay updated with current trends and regulations in social work practices and participate in ongoing training sessions.

Qualifications\n

Compensation and Benefits\n

The annual salary for this role ranges from $60,000 to $100,000, commensurate with experience and qualifications.
