Key Responsibilities\n

• Conduct thorough assessments to determine the needs of patients, enabling personalized treatment plans.

• Maintain clear and ongoing communication with supervisors as well as direct care staff, families, and guardians regarding a client’s progress and therapeutic approach.

• Develop and provide a tentative schedule of appointments, home visits, and observations to track patient progress effectively.
\n
• Coordinate home visits as required to facilitate in-home observations, conduct necessary chart reviews, and gather data for enhanced service delivery.
\n
• Implement and monitor the efficacy of program plans for each patient, ensuring methods align with best practices and individual client goals.
\n
• Actively participate in Interdisciplinary Team Meetings, providing valuable insights and updates on patient care as required.
\n
• Create and finalize monthly and quarterly progress notes that detail each client’s journey and outcomes.
\n
• Organize and provide training sessions to staff, ensuring competency in program plan implementation and related documentation processes.
\n
• Conduct follow-up interviews and home visits to ensure continued support for patients’ evolving needs.
\n
• Maintain accurate and accessible documentation to guarantee information is readily available to all relevant parties.
\n
• Serve as a liaison with the public and local mental health community to support development initiatives and outreach programs.
\n
• Participate in developing procedural guidelines for training materials, enhancing program efficacy and staff preparedness.

Patient Population / Client Focus\n

You will work with a diverse client base, including both adults and youth, who are seeking effective support for a variety of mental health challenges. Your experience with specialties such as family therapy, couples therapy, substance misuse, and trauma will be instrumental in guiding your therapeutic approach to facilitate healing and progress.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Tennessee.
\n
• Minimum of two years of experience in a counseling role, preferably within a clinical or community-based setting.

• Strong proficiency with Electronic Medical Record (EMR) software and related technologies essential for documentation and communication.
\n
• An in-depth understanding of legal and ethical standards as they pertain to mental health practice is critical.

• Excellent interpersonal, rapport-building, and communication skills that foster trust and effective client engagement.

• Proven ability to collaborate effectively within a healthcare team and contribute positively to a multidisciplinary setting.

Preferred Experience\n

• Specialized training or experience in family therapy, trauma-informed care, or addiction counseling is highly preferred.

Compensation and Benefits\n

Our competitive salary range is between $90,000 and $110,000 annually, commensurate with your experience and caseload availability.
