Key Responsibilities\n

• Conduct comprehensive assessments of patients to determine their counseling needs and to develop personalized treatment plans.

• Facilitate communication between supervisors, direct care staff, family members, and other stakeholders regarding patient progress and any necessary adjustments in care.

• Create and share a tentative schedule of appointments, home visits, and ongoing progress monitoring activities with supervisors.
\n
• Coordinate and conduct home visits to perform in-home observations, facilitate chart reviews, and engage in team interviews as required.
\n
• Ensure that treatment protocols are effectively implemented and adhered to throughout the care process.
\n
• Actively participate in Interdisciplinary Team Meetings and contribute insights and updates on patient conditions.
\n
• Document progress notes on a monthly and quarterly basis to track patient achievements and any areas needing attention.
\n
• Organize and provide training for staff and team members in the implementation of therapy techniques and documentation practices.
\n
• Perform follow-up interviews, home visits, and additional observations to ensure ongoing support for patients’ evolving needs.

• Maintain meticulous records to ensure that information is accessible to relevant parties, fostering transparency and continuity of care.

• Serve as a liaison with the public and the greater mental health community to support program development initiatives.

• Assist in developing training materials aimed at enhancing program effectiveness for diverse populations.

Patient Population / Client Focus\n

The LPC will work with a broad demographic ranging from children to adults, providing care for individuals navigating various challenges including stress, anxiety, trauma, substance use, and relationship issues. This role allows for the application of therapeutic modalities such as cognitive-behavioral therapy, family therapy, and trauma-informed care, ensuring a well-rounded approach to individual and family mental health.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related discipline.

• Current, unrestricted licensure as a Professional Counselor (LPC) in the state of Georgia.

• A minimum of two years’ experience in a counseling capacity, particularly within clinical settings.

• Demonstrated expertise in areas such as family therapy, couples therapy, trauma, or substance misuse is highly desirable.
