Key Responsibilities\n

• Collaborate with Reentry Social Workers to develop effective discharge planning services that ensure proper transition and support for patients.

• Prepare and complete all necessary statistical reports to track treatment outcomes and facilitate quality improvement processes.
\n
• Work closely with the Department of Correction to ensure timely access to services and appropriate housing and treatment dispositions for patients within the system.
\n
• Lead and coordinate group therapy sessions for patients in mental health observation units and other designated areas, focusing on therapeutic support and skill-building.
\n
• Conduct thorough mental health evaluations, crisis interventions, assessments, and follow-up care to monitor patient progress and adjust treatment plans as needed.
\n
• Maintain meticulous documentation of patient interactions, treatment plans, and progress, ensuring compliance with both legal and ethical standards.
\n
• Participate actively in clinical supervision with a Clinical Supervisor at least once a week to foster personal and professional development.
\n
• Engage in mental health and medical rounds, providing holistic insights into patient care strategies and collaborating with interdisciplinary teams.
\n
• Facilitate case conferences to discuss treatment plans for patients receiving mental health services or being evaluated for care.
\n
• May supervise social work interns or student volunteers, guiding them in best practices around patient care and clinical skills.

This position primarily involves working with hospice patients, their families, and individuals facing end-of-life challenges. You will address a range of emotional and psychological needs while enhancing coping mechanisms and providing grief support. Qualifications\n
