Key Responsibilities\n

• Conduct assessments to understand the emotional and clinical needs of clients and their families, drawing on available psychosocial history.

• Engage in individual and family counseling sessions to foster resilience and coping strategies for complex issues such as grief, loss, and psychological distress.

• Manage case documentation using our electronic health record (EHR) system, ensuring accurate and timely updates to client information.

• Provide guidance and collaboration with the practitioner team to identify and mitigate potential risks, particularly in crisis situations.

• Lead interventions, including escalation procedures to emergency services when necessary, ensuring client safety and adherence to mandatory reporting policies.

• Employ evidence-based practices to monitor outcomes, actively seeking feedback from clients and their families, and making necessary adjustments to care plans.

• Stay informed about current safeguarding policies, and implement them into daily operations, addressing any challenges that may arise.

Patient Population / Client Focus\n

This role is centered around serving a unique patient demographic within a hospice context. You will work with children, youth, and adults grappling with end-of-life transitions, providing therapeutic support during these critical moments. Clients often experience varying degrees of emotional distress related to chronic illness, terminal diagnoses, and the grief associated with losing loved ones. Your ability to connect, empathize, and facilitate conversations about death and dying will significantly impact their overall experience.

Qualifications\n

• Master's Degree in Social Work (MSW) from an accredited institution.

• Possession of an active Licensed Clinical Social Worker (LCSW) designation is mandatory.

• A minimum of two years of experience in a behavioral health setting, ideally with a focus on hospice, palliative care, or similar environments.

• Demonstrated ability in conducting psychosocial history interviews and case management evaluations.

• Experience in crisis intervention and suicide risk assessments.

Compensation and Benefits\n
