Key Responsibilities\n

• Perform continuous and thorough assessments to identify patient needs and create personalized care plans that address their unique circumstances.

• Communicate consistently with supervisors, healthcare team members, and family or guardian networks to provide updates on client progress and collaborative planning.

• Develop and maintain a tentative schedule that includes appointments, home visits, and monitoring sessions, optimizing time management for effective patient engagement.

• Coordinate necessary home visits that meet specific requirements for in-home observations, including chart reviews and staff interviews, to ensure comprehensive care.

• Ensure the effective implementation of program plans, adhering to established protocols and adjusting as necessary to meet patient needs.
\n
• Actively participate in Interdisciplinary Team Meetings and other relevant discussions to contribute insights and collaborate on patient care strategies.
\n
• Document and complete monthly and quarterly progress notes to accurately track patient development and care outcomes.
\n
• Organize and conduct training for staff and team members, ensuring competence in program plan implementation and accurate documentation practices.

• Provide follow-up interviews, home visits, and observations to enhance continued support and address evolving patient needs.

• Maintain detailed records that ensure information is accessible to all interested parties, facilitating informed care decisions.

• Serve as a liaison with the public and mental health community to advance organizational development goals and outreach efforts.

• Assist in the creation of training materials that enhance program delivery and improve patient outcomes through innovative practices.

Qualifications\n

• A solid foundation in family therapy, couples therapy, substance misuse, or trauma recovery is highly preferred.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 90,000 to 100,000 annually, based on experience and caseload management.
