Key Responsibilities\n

• Proactively engage with participants to identify and address concerns before they escalate, thereby minimizing reliance on crisis services.

• Maintain accurate and timely documentation of crisis alerts to ensure seamless coordination with county crisis response services.

• Facilitate transitions for participants needing hospital or inpatient services, coordinating with medical staff to ensure thorough discharge and care continuity.

• Conduct regular telehealth sessions, utilizing evidence-based practices to provide counseling tailored to the diverse psychological and emotional needs of clients.

• Participate in team meetings for case reviews and care coordination efforts, sharing insights that contribute to service improvement.

Patient Population / Client Focus\n

The ideal candidate will work with a diverse client base, encompassing a wide spectrum of psychiatric, social, and emotional challenges. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Current and active licensure as either a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC).

Compensation and Benefits\n

This role offers a competitive salary range of $100,000 to $105,000 annually, based on caseload and experience.
