Key Responsibilities\n

• Conduct ongoing assessments to evaluate the needs and treatment progress of patients, ensuring a tailor-made approach to care.

• Collaborate closely with multidisciplinary teams, including family members, guardians, and collateral contacts, to communicate updates regarding the client's progress and treatment plans.

• Develop and provide a tentative schedule for supervision of appointments, home visits, and observations needed to effectively monitor treatment adherence.
\n
• Coordinate and execute home visits aligned with the requirements for assessments, including chart reviews, staff interviews, and data analysis to understand patient environments better.
\n
• Guarantee that the implementation of program plans is effective and aligned with the established goals for patient care.
\n
• Participate actively in Interdisciplinary Team Meetings to ensure comprehensive and coordinated care delivery.
\n
• Prepare monthly and quarterly progress notes to document treatment evolution and outcomes.
\n
• Facilitate training sessions to ensure all team members are well-prepared to implement the program plan and maintain accurate documentation.
\n
• Provide continued support through follow-up interviews and home visits to address clients’ ongoing needs.
\n
• Maintain organized records and documentation to assure availability and confidentiality for necessary stakeholders.
\n
• Act as a liaison between the clinic and the community, fostering relationships to enhance awareness of mental health initiatives.

• Assist in developing training materials aimed at improving clinic programs and therapeutic practices.

Patient Population / Client Focus\n

In this position, you will engage with a diverse patient population, including both adults and youth. You will be tasked with delivering compassionate counseling services in various areas, including family dynamics, substance misuse, and trauma recovery. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field, accompanied by active and unrestricted licensure as a Professional Counselor in the state of Colorado.
