Key Responsibilities

• Conduct comprehensive evaluations, including intake history and assessments, to formulate strategic treatment plans tailored to individual patient needs.
• Communicate evaluation outcomes and treatment protocols effectively to patients and their families, fostering collaborative goal-setting.
• Implement and adhere to established treatment protocols, demonstrating proficiency in adjusting interventions based on patient progress and healthcare trends.
• Consult with interdisciplinary healthcare teams on patient care modifications, ensuring timely responses to any changes in condition.
• Execute thorough discharge planning processes, documenting all care activities succinctly, including daily progress notes and interim summaries.
• Manage patient caseload with effective time management skills, ensuring appointments and documentation are completed as per productivity expectations.
• Contribute to departmental goals by participating in strategic initiatives and compliance with regulatory standards.
• Pursue ongoing professional development opportunities, serving as a clinical mentor to students and interns in physical therapy.
• Enhance customer service experiences by demonstrating empathy and adaptability, striving to remove barriers to access to care.
• Engage in initiatives aimed at improving access to services and quality of care in alignment with organizational objectives.

Patient Population / Client Focus

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid Physical Therapy license in the State of New Jersey.

Preferred Experience

While we do not require prior work experience, candidates with a demonstrated background in outpatient physical therapy will be given preference. Familiarity with electronic medical record (EMR) systems and experience with rehabilitation methodologies can enhance your candidacy.
