Key Responsibilities\n

As a Licensed Clinical Professional Counselor at Crystal Lake Families & Kids Services, you will:

• Collaborate with primary care providers to recognize, treat, and manage mental health and psychosocial conditions, ensuring a cohesive approach to patient care.

• Conduct thorough patient intakes that encompass diagnostic and functional evaluations, providing informed recommendations to primary care providers for treatment goals and plans.

• Deliver brief, targeted therapeutic interventions for patients requiring mental health support, tailoring your approach to meet individual needs.
\n
• Advise primary care providers on appropriate referrals to specialty mental health services when necessary, ensuring that patients receive optimal care.
\n
• Initiate and manage follow-up interactions to assess patient progress, making necessary adjustments to treatment strategies as indicated.
\n
• Develop relapse prevention plans for high-risk patients, playing a crucial role in helping them maintain stable functioning.
\n
• Monitor the delivery of behavioral health services, coordinating with external treatment providers when appropriate and with patient consent.
\n
• Document patient progress, diagnostic insights, and all relevant information in treatment records, maintaining high standards of clinical documentation.

• Engage with patients to promote self-management skills, empowering them to take an active role in their treatment and recovery journey.

• Provide insights and recommendations to clinic management and interdisciplinary team members about enhancing behavioral health services.

• Assist patients with compliance regarding medical treatments initiated by primary care providers, and offer coping strategies for any medication side effects.

Qualifications\n

The successful candidate will possess:

• A minimum of a Master’s Degree in Clinical Counseling.

• A thorough knowledge of the personal and social effects of mental health issues on individuals and families.

Preferred Experience\n

While not required, bilingual proficiency in English and Spanish is preferred, as it enhances our ability to serve a broader range of patients. Having experience in telehealth or remote counseling practices may also be beneficial.

Compensation and Benefits\n
