Key Responsibilities\n

• Conduct comprehensive assessments to evaluate clients' mental health needs, identifying issues related to emotional, psychological, and social factors.

• Develop and implement individualized treatment plans that align clients' personal goals with evidence-based therapeutic interventions.

• Provide psychotherapeutic counseling services via telehealth or in-person sessions, assisting clients in navigating life transitions, stressors, and challenges.

• Advocate for client access to community resources such as support groups, educational workshops, and wellness programs to foster continued growth and independence.

• Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and social workers, to ensure coordinated and comprehensive care for each client.

• Monitor clients' progress through regular check-ins and adjust treatment plans as necessary based on evolving needs and circumstances.

• Maintain thorough and accurate documentation of client interactions, assessments, and treatment plans in compliance with organizational and regulatory standards.

• Participate in team meetings and case consultations to share insights, discuss challenges, and strategize effective interventions for complex cases.

Patient Population / Client Focus\n

This role primarily focuses on adult clients who may face a variety of mental health challenges, including anxiety, depression, grief, relationship issues, and other psychological conditions. You will work closely with clients from diverse backgrounds, ensuring culturally competent care that respects individual differences and promotes inclusivity. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current license as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.
