Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observations, and reviews of medical records to evaluate emotional, social, and environmental factors affecting patients’ recovery.

• Provide empathetic counseling and emotional support to both patients and their families, assisting them through adjustment issues related to illness or disability.

• Collaborate closely with the multidisciplinary rehabilitation team to develop and implement individualized treatment plans that consider each patient’s unique psychosocial needs.

• Facilitate access to community resources, including support groups, vocational rehabilitation programs, and housing assistance, ensuring that patients can transition smoothly back into their communities.

• Educate patients and families on available post-rehabilitation services and community resources, enhancing their knowledge and empowerment.

• Participate actively in discharge planning, ensuring seamless transitions to home or community settings.

Patient Population / Client Focus\n

You will work with a diverse patient population, including those recovering from severe medical conditions or injuries. Your primary focus will be on providing emotional and practical support that aligns with each patient's rehabilitation goals. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Valid LCSW licensure in the state of Colorado.
