Work Environment

This position is onsite, where you will work directly with patients in their homes, an approach that allows for personalized and effective care in a comfortable setting. The typical work schedule is Monday through Friday, from 8:00 AM to 5:00 PM, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients with various functional limitations and disabilities, establishing tailored treatment plans to address their specific needs.
• Adhere to state practice acts and ensure compliance with laws governing physical therapy practice, while providing supervision to appropriate personnel.
• Perform comprehensive evaluations, including a review of medical history, implementation of relevant assessments, and thorough documentation of results and recommendations.
• Implement effective treatment protocols based on evaluation findings and evolving patient conditions, adapting strategies to maximize healing and rehabilitation.
• Consult with other healthcare professionals when necessary, ensuring that changes in a patient's condition are promptly recognized and addressed through modified care plans or additional medical consultations.
• Engage in comprehensive discharge planning, ensuring proper follow-up care is in place and maintain thorough documentation that illustrates the patient's course of treatment, including evaluations, progress notes, and discharge summaries.
• Participate in ongoing professional training to ensure adherence to corporate and clinical training requirements and compliance with all regulatory standards.
• Deliver timely and quality care while balancing departmental productivity targets, conducting treatments as assigned, and ensuring a superior patient experience.
• Attend and actively participate in conferences, staff meetings, and in-service training sessions to remain current in best practices and clinical advancements.
• Actively engage in initiatives aimed at removing barriers to patient access, emphasizing the importance of data measurement and care accessibility.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including those recovering from surgeries, injuries, and chronic conditions. The home care environment allows you to build strong therapeutic relationships with patients, fostering trust and promoting adherence to rehabilitation programs. Your ability to customize treatment plans based on individual patient conditions will be crucial in achieving positive health outcomes.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited PT program.
• A valid state license to practice as a Physical Therapist.
• A minimum of two years of practical experience in the field, preferably in a home health or outpatient setting.
• Current CPR certification.
• Superior interpersonal skills with demonstrated ability to build rapport with patients and work collaboratively within a team.

Preferred Experience

• Experience in home health settings or similar care environments.
• Familiarity with digital health solutions or applications in patient care.
• Prior involvement in multidisciplinary care teams, enhancing collaborative care approaches.

Compensation and Benefits
