Key Responsibilities\n

• Provide both individual and group counseling, focusing on challenges such as adjustment issues, grief, and coping mechanisms.

• Develop discharge plans in collaboration with the healthcare team to ensure continuity of care and connect patients with necessary community resources post-discharge.

• Facilitate crisis intervention and provide immediate emotional support to patients and families facing urgent psychosocial challenges.

• Participate in interdisciplinary team meetings to discuss patient progress and refine treatment plans, ensuring optimal health outcomes.

• Maintain meticulous documentation of patient interactions and treatment plans in compliance with both hospital policies and legal guidelines.

Qualifications\n

Compensation and Benefits\n
