Key Responsibilities

• Provide high-quality, patient-centered care by managing the entire therapeutic process for a diverse caseload of patients in collaboration with attending physicians and the wider care team.
• Conduct thorough patient assessments to evaluate physical abilities, functional limitations, and rehabilitation needs, using the insights gathered to develop individualized care plans.
• Implement and direct evidence-based interventions focusing on optimizing recovery and enhancing physical capabilities.
• Execute evaluations in alignment with applicable laws and ethical standards, ensuring the safety and legality of all therapeutic actions.
• Instruct patients and their families or caregivers in appropriate exercise regimens and the use of therapeutic devices, ensuring they feel confident and empowered in their healing journeys.
• Prioritize and manage your caseload effectively, determining daily staffing needs and streamlining protocol adherence.
• Document all patient interactions and progress in accordance with organizational requirements, maintaining transparent communication within the care team.

Patient Population / Client Focus

In this role, you will engage with a broad spectrum of clients, from pediatric to geriatric patients, each with distinct therapeutic needs. You will address conditions ranging from post-surgical rehabilitation to chronic pain management and developmental issues. Your expertise will be vital in fostering positive outcomes and enhancing the quality of life for each patient in their unique home environments.

Qualifications

• Must possess a valid license to practice Physical Therapy in the state of Florida.
• A Doctorate in Physical Therapy (DPT) is required.
• Basic Cardiac Life Support (BCLS) certification must be obtained within the first 30 days of employment.
• While experience in the field is preferred, newly licensed PTs are encouraged to apply and will be supported through mentorship and guidance.

Preferred Experience

• Prior experience in home health, outpatient rehabilitation, or related settings.
• Familiarity with electronic medical record (EMR) systems and telehealth services is advantageous but not mandatory.
• Knowledge of current evidence-based practice guidelines and therapeutic modalities relevant to physical therapy.

Compensation and Benefits
