Work Environment\n

This role will be conducted in a remote setting, allowing for flexibility while ensuring that you remain connected with our clients and the larger multidisciplinary team. You will engage with clients virtually, using telehealth tools and electronic health record (EHR) systems to maintain high standards of care. Our work hours are typically Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend availability required as client needs dictate.

Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be responsible for:

• Conducting comprehensive assessments of seniors' psychosocial needs, focusing on emotional, mental, and social health factors.

• Developing and implementing personalized care plans in collaboration with clients and their families to meet specific goals and enhance quality of life.

• Providing individual and family counseling, offering emotional support that helps clients cope with transitions, challenges, and life events.

• Advocating for clients to ensure they have access to necessary resources and services within the community.

• Working closely with an interdisciplinary team of nurses, caregivers, and healthcare professionals to coordinate holistic care plans.

• Regularly visiting clients (virtually or in-person as needed) to monitor their well-being and safety and address any concerns regarding their living situations.

• Facilitating family meetings to foster communication and resolution of conflicts among those involved in a senior's care.

• Providing education to clients and their families on available community resources, support groups, and programs aimed at enhancing social engagement.

• Maintaining accurate, up-to-date client records and ensuring compliance with ethical standards and regulatory requirements.

Patient Population / Client Focus\n

Your primary focus will be on seniors, who often face unique challenges, including chronic illness, mental health issues, social isolation, and the need for comprehensive support systems. Understanding the complexities of aging will be key to effectively advocating for and assisting your clients and their families.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active and unrestricted state licensure as a Master Social Worker (LMSW) in New Jersey.

• A minimum of two years of relevant experience in the field of social work, preferably within a community health or geriatric environment.

• Proven knowledge of social work principles, aging-related issues, and community resource availability.

• Strong assessment, counseling, and advocacy skills.

• Exceptional communication and interpersonal abilities to effectively collaborate with a variety of professionals and clients.

Compensation and Benefits\n

Our competitive compensation package ranges between $60,000 and $100,000 annually, depending on experience and qualifications.
