Key Responsibilities\n

• Conduct thorough diagnostic assessments to evaluate clients’ mental health and create individualized treatment plans that effectively address their unique needs.

• Deliver engaging and effective individual and group counseling sessions, focusing on evidence-based practices and trauma-informed care methodologies.

• Maintain meticulous records, including clinical and progress notes, ensuring compliance with organizational policies and relevant regulations.

• Interface regularly with healthcare providers and physicians to communicate patient progress, upcoming needs, concerns, and any emergent issues requiring attention.

• Provide crisis intervention and supportive counseling, assisting clients in navigating emotional and behavioral challenges.

• Facilitate care coordination and assist clients with comprehensive discharge planning, ensuring a smooth transition to ongoing care.

• Consult with families and support systems as needed to enhance treatment effectiveness and to assist in discharge preparation.

As a Licensed Professional Counselor at Strength Path Wellness Center, you will work with a diverse client population, including individuals facing trauma, anxiety, depression, and behavioral disorders. Qualifications\n
