As a home care specialist, you will work directly with patients in their homes, tailoring your approach to their unique living situations and health challenges. Key Responsibilities

• Evaluate, plan, and implement individualized treatment programs for patients referred by physicians, based on comprehensive assessments of their physical abilities and needs.
• Utilize advanced techniques and modalities, including manual therapy and therapeutic exercise, to facilitate recovery in patients with orthopedic, pediatric, neurologic, and sports medicine conditions.
• Deliver skilled therapeutic care in the patient's home, ensuring treatments are consistent with the physician's plan of care and tailored to each individual's living environment.
• Educate patients and their caregivers on condition management and rehabilitation exercises to promote recovery and prevent future injuries.
• Maintain accurate records of patient progress and treatment outcomes in accordance with regulatory requirements and company policies.
• Meet established utilization and productivity goals while contributing to the team's overall effectiveness through excellent customer service.

Patient Population / Client Focus

In this role, you will serve a diverse range of patients, with a primary focus on individuals requiring orthopedic rehabilitation. Your expertise will be crucial in planning and executing rehabilitation strategies that enhance recovery outcomes for patients of all ages, from pediatric to geriatric populations. Qualifications

• A Bachelor's Degree in Physical Therapy from an APTA or CAPTE-approved program is required.
• Current state licensure in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Excellent verbal and written communication skills strength, allowing you to build trusting relationships with patients and their families.

Preferred Experience

• Experience in a home care setting, particularly in treating orthopedic conditions.
• Strong background in evidence-based practice and familiarity with client management systems.
• Aptitude for utilizing various therapeutic techniques specific to orthopedic rehabilitation.

Compensation and Benefits
