Key Responsibilities\n

As a Licensed Master Social Worker at Life Stage Counseling, you will:

• Perform comprehensive assessments of seniors’ psychosocial needs, addressing emotional, mental, and social factors to develop individualized care plans.

• Work closely with clients and their families to create achievable goals that cater to their unique aspirations and challenges.

• Provide counseling and emotional support, guiding seniors and their families through transitions such as relocation to assisted living or dealing with the loss of a loved one.

• Advocate for clients to ensure they have access to necessary community resources, including healthcare, support groups, and social services.

• Conduct home visits, regular check-ins, and telehealth appointments to evaluate client well-being and living conditions, offering immediate interventions as needed.

• Facilitate family meetings to improve communication and resolve conflicts concerning the care of the senior clients.

• Educate clients about available resources to promote social engagement and enhance their overall quality of life.

• Maintain accurate records of assessments, care plans, and treatment notes using electronic health record (EHR) systems.

• Stay updated on best practices in social work and compliance with state regulations to uphold the highest ethical standards in practice.

Qualifications\n

• Master’s degree in Social Work from an accredited school.

• Active state licensure as a Licensed Master Social Worker ( LMSW ) in Florida.

• A minimum of two years of professional experience in a social work role, preferably within geriatric care or community services.

• Sound knowledge of social work principles, as well as a clear understanding of aging-related issues and community resources.

• Strong counseling, assessment, and problem-solving abilities that allow for effective intervention and client support.

Preferred Experience\n

While a minimum of two years of relevant experience is required, candidates with prior involvement in a community health environment or specialized training in geriatric social work are encouraged to apply. Experience in crisis intervention and family mediation is also highly desirable.
