As part of a hospice center, you will collaborate closely with a multidisciplinary team, including healthcare providers and social workers, to deliver cohesive and compassionate care to our clients. Regular team meetings, training sessions, and engagement with community stakeholders will be integral to your work.

Key Responsibilities\n

• Conduct thorough assessments to identify social, emotional, and environmental challenges faced by clients, ensuring a comprehensive understanding of their needs.

• Design and implement individualized service plans that detail clear goals, interventions, and a system for tracking progress over time.

• Provide high-quality counseling services to individuals and families facing emotional distress, including techniques such as CBT and narrative therapy to help clients navigate their issues.

• Work collaboratively with clients to connect them with essential community resources, such as housing assistance, support groups, and substance abuse programs.

• Engage in crisis intervention when necessary, performing risk assessments, safety planning, and providing immediate support to individuals in distress.

• Establish partnerships with community agencies and organizations to advocate for and coordinate services for your clients.
\n
• Participate in community outreach efforts to raise awareness of mental health services and advocate for the needs of our client population.

• Attend regular case conferences and team meetings, and take part in ongoing professional development opportunities to maintain best practices in social work.

Patient Population / Client Focus\n

Your work will primarily focus on individuals and families facing complex life challenges, particularly those receiving care within a hospice setting. You will provide empathetic and culturally competent support to clients grappling with grief and loss, navigating significant life transitions, and striving for emotional healing in times of crisis.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• A strong understanding of social and economic justice issues relevant to the communities we serve.

• Excellent interpersonal communication skills, cultural competence, and a deep commitment to client-centered care.

• Proficiency in trauma-informed care practices and an awareness of community resources available for client support.

Preferred Experience\n

Compensation and Benefits\n
