Key Responsibilities

• Engage actively in treatment planning by collaborating with a multidisciplinary healthcare team to ensure comprehensive and effective discharge plans are in place based on detailed psychosocial assessments.
• Conduct thorough assessments of patients' mental and emotional health, developing tailored interventions that align with their specific needs and circumstances.
• Maintain meticulous and timely documentation including clinical notes and progress reports in accordance with department protocols and electronic medical record systems.
• Provide counseling and crisis intervention services to patients facing emotional and behavioral challenges, applying evidence-based practices to foster their resilience and coping skills.
• Consult with family members and primary caregivers to coordinate support systems, ensuring that discharge planning and ongoing treatment align with familial support structures.

Patient Population / Client Focus

In this role, you'll be directly supporting patients in the hospice care setting, typically working with individuals experiencing end-of-life transitions as well as their families. The ideal candidate will be sensitive to the complexities surrounding end-of-life issues, pain management, and grief, and must possess the ability to provide empathetic support to both patients and their loved ones.

Qualifications

• A Master’s degree in Social Work or Counseling from an accredited institution is required.
• An active and unrestricted LICSW license in the state of Washington.
• A minimum of three years of unsupervised post-licensure experience in a clinical setting is essential.
• Proficiency in using electronic medical records, word processing software, spreadsheet applications, and email systems to maintain records and documentation.

Preferred Experience

• Experience in hospice care, palliative care, or working with terminally ill patients is highly valued.
• Demonstrated ability to support diverse age groups across varying backgrounds, with an emphasis on treating holistic needs.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $105,000 per year, reflective of your experience and qualifications. Additionally, benefits include:
