Key Responsibilities

• Conduct thorough behavioral health diagnostic evaluations and crisis assessments to identify client needs and develop appropriate treatment plans.
• Utilize standardized assessment tools to gauge cognitive, behavioral, and mental health outcomes for patients.
• Deliver evidence-based psychotherapy through individual sessions and group therapy, ensuring a safe space for patients to discuss their challenges and progress.
• Provide ongoing support to clients, their families, and caregivers by offering resources and guidance as needed.
• Maintain regular communication with clients through telehealth platforms, offering both virtual consultations and telephone support.
• Facilitate care coordination by collaborating with primary care providers, specialists, and home health aides to ensure comprehensive patient management.
• Implement case management strategies to assist clients and families in navigating healthcare systems and accessing necessary services.

Patient Population / Client Focus

Your primary focus will be working with an adult population facing a range of challenges, including mental health disorders, psychosocial issues, and significant life transitions. This role emphasizes building strong, longitudinal relationships with clients, fostering trust and a sense of safety, which are crucial for effective therapy and support.

Qualifications

• A Bachelor’s degree in Social Work (Master’s preferred) from an accredited program.
• An unrestricted, current state license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge and competency in assessment, diagnosis, and treatment of behavioral health issues.
• Ability to work independently while being a supportive team player.
• Excellent communication and interpersonal skills, with a strong ability to engage clients and families empathetically.
• Proficient in utilizing telehealth platforms for patient evaluations and therapy sessions.

Preferred Experience

• Experience in a community health setting, particularly working with diverse and underserved populations.
• Certification in psychotherapy techniques and methodologies.
• Familiarity with case management practices and care coordination.
• Experience participating in interdisciplinary teams aimed at holistic patient care.
• Strong organizational skills with the ability to manage multiple cases effectively and efficiently.

Compensation and Benefits

We offer a competitive salary range of USD 115,000 to 130,000 annually, commensurate with your experience and qualifications. Additionally, our comprehensive benefits package includes:
