This position is based in Newark, New Jersey, where you will work autonomously in patients' homes to deliver quality physical therapy services. Key Responsibilities

• Direct the application of physical therapy interventions to prevent disabilities, alleviate pain, and restore optimal muscle functionality, in accordance with individualized treatment plans.
• Oversee and collaborate with physical therapy assistants to ensure the accurate implementation of care plans tailored to each client's unique needs.
• Conduct thorough evaluations to assess individuals' functionality and devise rehabilitation strategies by administering diagnostic tests on muscle, nerve, joint, and functional abilities.
• Assess patients' home environments to make appropriate recommendations that maximize their independence and comfort during rehabilitation.
• Administer varied physical therapy treatments, including manual exercises, traction, massage, and application of physical agents, to support patient recovery.
• Evaluate and recommend modifications for prosthetic and orthotic devices, ensuring they meet the needs of our clients.
• Participate in the clinical review process by maintaining up-to-date patient records, including initial evaluations, care plans, progress notes, and discharge summaries, and ensure compliance with OASIS documentation standards.
• Engage in interdisciplinary team meetings to discuss and refine treatment strategies and participate in creating holistic care approaches that align with patient goals.
• Provide education and training to clients and families on the use of physical therapy devices and their roles within the larger therapeutic plan.
• Complete on-site functional assessments, updating care plans and evaluating the use of community resources as needed.
• Adhere to company policies and state laws concerning documentation submission, patient care, and supervision of physical therapy assistants.

Patient Population / Client Focus

The primary focus of this role will be on patients requiring orthopedic care. This includes individuals recovering from surgeries, injuries, or those living with chronic pain. Our multidisciplinary approach ensures that we cater to each patient's emotional and psychological needs by providing support tailored to their situation, thereby enhancing their overall recovery process.

Qualifications

• Bachelor's degree from an accredited physical therapy program is required.
• A minimum of one year of clinical experience as a physical therapist, preferably in an acute care or home care setting.
• Current physical therapist licensure in good standing in the State of New Jersey.
• Active CPR certification and a valid driver's license with appropriate automobile insurance.
• Creative problem-solving skills and strong leadership capabilities.
• Ability to assess situations quickly and make pivotal decisions regarding patient care.

Preferred Experience

• Experience in home health care settings is preferred, as it allows for a better understanding of delivering therapy in a non-clinical environment.
• Previous experience working collaboratively within a multidisciplinary healthcare team will be beneficial.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, based on experience and qualifications.
