Full-Time Licensed Clinical Social Worker for Hospice Care

Work Environment

This position is fully remote, offering flexibility while maintaining a strong connection with clients through telephone and video consultations. You will collaborate with a dedicated team of healthcare professionals, including nurses, physicians, and fellow social workers, to coordinate patient care seamlessly. Key Responsibilities

• Conduct thorough assessments of clients’ needs and gather critical information to develop personalized care plans.
• Provide direct support to clients via phone and video consultations, helping them navigate issues related to neglect, child abuse, mental health, and domestic violence.
• Offer crisis intervention services and emotional support to families facing challenging circumstances.
• Facilitate referrals to external agencies and services to ensure comprehensive care for clients and their families.
• Maintain meticulous records of client interactions and progress, preparing reports that adhere to organizational standards.
• Engage in regular supervision, training, and team meetings to enhance skills and stay updated on best practices.
• Actively participate in community outreach initiatives to promote awareness and access to mental health resources.
• Be vigilant for signs of child abuse and other forms of maltreatment, reporting findings in accordance with legal and ethical standards.

Patient Population / Client Focus

In this role, you will work primarily with terminally ill patients and their families, providing essential emotional and psychological support during a profoundly challenging period. You will address a variety of issues related to grief, loss, and mental health, ensuring that each individual receives not only immediate care but also ongoing support in coping with their situation. Your client interactions will involve a high level of empathy and understanding, directly contributing to improved patient outcomes.

Qualifications

• A Master’s degree in social work, psychology, or a related field is required.
• Possession of a valid clinical social work license (LCSW) in the state of Texas.
• Strong ability to listen actively and communicate effectively with clients, demonstrating empathy and non-judgment.
• Proficiency in various software applications, including Microsoft Word, Excel, PowerPoint, and Outlook, as well as telehealth platforms.
• Exceptional organizational skills and the ability to conduct thorough assessments and treatment planning.
• Knowledge of crisis intervention strategies and techniques.
• A valid driver’s license and willingness to travel for community outreach as needed.

Preferred Experience

• Experience working in hospice or palliative care settings, focusing on end-of-life issues.
• Prior experience managing cases involving clients affected by domestic violence or substance abuse is highly regarded.
• Familiarity with community resources and social service agencies in the Dallas, Texas area.

Compensation and Benefits

The salary for this position ranges from USD 90,000 to 100,000 per year, commensurate with experience and qualifications.
