Key Responsibilities\n

• Conduct comprehensive psychosocial assessments upon patient admission and as clinically indicated, utilizing established social work theories and techniques to identify emotional, social, and physical needs while addressing barriers to care.

• Collaborate with interdisciplinary teams to develop and implement tailored psychosocial treatment plans. This includes seamlessly coordinating referrals to specialty services and ensuring continuity of care through diligent progress note documentation.

• Provide supportive counseling and crisis intervention to patients and their families, helping them navigate the emotional and psychological challenges associated with medical conditions and transitions.

• Manage an independent caseload with varying levels of clinical acuity, demonstrating effective case management and problem-solving skills.

• Participate actively in case finding initiatives to identify patients and families requiring psychosocial support, attending team conferences, and interdisciplinary rounds as necessary to enhance care coordination.

• Render psychoeducation to patients and families regarding diagnoses and treatment, facilitating understanding and adaptation to medical challenges and changes in care needs.

• Contribute to quality assurance efforts and community improvements related to social work services, participating in the review and development of policies and procedures.

• Make mental health diagnoses when appropriate, enrich treatment plans, and oversee clinical supervision for MSWs and student interns.

• Prepare and maintain electronic medical records, ensuring compliance with health system policies while providing written and verbal reports relevant to care management.

Qualifications\n
