Key Responsibilities\n

• Collaborate with healthcare team members to develop comprehensive treatment plans based on psychosocial assessments of our clients, ensuring alignment with recognized social work interventions.

• Conduct thorough patient assessments and implement targeted interventions to address individual needs, including emotional and behavioral challenges.

• Provide crisis intervention and counseling services to clients, utilizing your clinical expertise to support their psychological health during critical moments.

• Maintain accurate and timely clinical documentation, including progress notes and discharge summaries, in compliance with department protocols and regulatory standards.

• Engage with family members and the patient’s support system, coordinating efforts around treatment strategies and aftercare plans to facilitate smooth transitions post-discharge.

Qualifications\n
