Key Responsibilities\n

• Establish a quick rapport with patients and their families, utilizing active listening and effective communication to ensure clarity in interactions.

• Conduct evidence-based screenings for mental health conditions, including depression, anxiety, and substance abuse, while assessing factors that may impact patient health.

• Create and manage a patient caseload, providing follow-up interventions that are compassionate and tailored to each individual’s needs.

• Utilize electronic medical records for thorough documentation, ensuring that all patient interactions are recorded within 48 hours.

• Develop integrated care plans that coordinate patient services, facilitating communication between patients, families, and healthcare providers.

• Safeguard patient privacy and confidentiality in all documentation and communications, adhering to HIPAA regulations.

• Participate in individual and group supervision sessions, case conferences, and departmental meetings to enhance personal practice and contribute to team objectives.

• Respond promptly to calls from patients, family members, and providers, demonstrating a high level of care and concern for patient needs.

Qualifications\n

Compensation and Benefits\n
