Key Responsibilities\n

• Conduct one-on-one counseling sessions to assess client needs and establish therapeutic goals, preferences, and treatment options.

• Design and implement personalized rehabilitation plans that consider each client’s unique education, skills, physical capabilities, and aspirations. Adjust plans as necessary to respond to client progress.

• Collaborate with a team of healthcare professionals, including physicians and occupational therapists, to optimize client rehabilitation strategies.
\n
• Maintain comprehensive case files that include patient information, eligibility verification, documentation of services rendered, and detailed narratives of client interactions.

• Track and document client achievements regularly to assess the effectiveness of interventions, modifying treatment plans to ensure continued progress.

• Maintain ongoing communication with clients during job training and placement efforts, promptly addressing any challenges or concerns they may encounter.

• Engage with clients' families to understand their needs better and support their loved ones effectively throughout the counseling process.

• Conduct timely assessments and prepare reports reflecting client needs and treatment outcomes.

• Provide referrals to external resources and services to facilitate comprehensive support.

The clients you will serve are diverse, presenting with a range of mental health challenges, including anxiety disorders, mood disorders, and adjustment issues. Your expertise will be crucial in empowering clients, many of whom are navigating significant life changes or seeking to reintegrate into society following treatment or life transitions.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Psychology, or a related field is required.

• A valid Licensed Professional Counselor (LPC) license is mandatory.

• Demonstrated experience working with clients facing mental health challenges within residential or outpatient settings.

• Strong clinical competencies in therapeutic modalities such as motivational interviewing, cognitive-behavioral therapy, and dialectical behavior therapy.

Preferred Experience\n
