Key Responsibilities\n

• Conduct comprehensive assessments of each patient’s needs, background, and abilities, then develop tailored treatment plans that support symptom resolution and enhance their overall functioning.

• Offer individual, family, couple, and group psychotherapy sessions, utilizing evidence-based therapeutic techniques to support clients through their end-of-life experiences.

• Collaborate closely with the psychiatry team and other healthcare professionals to co-create effective program planning that addresses the diverse needs of our patients.

• Continuously evaluate the biopsychosocial needs of patients, consistently updating treatment plans to reflect any changes in their conditions or needs.

• Provide psychotherapies and case management services while also offering essential input regarding the care of program patients.

The primary clientele for this role includes senior individuals within a hospice setting, who may be experiencing various psychological challenges such as anxiety, depression, and grief. Qualifications\n

• Active Licensed Clinical Social Worker (LCSW) certification in Pennsylvania.

Compensation and Benefits\n
