Work Environment

This position requires an onsite presence, where you will work directly within the homes of clients, providing a comfortable and familiar environment that is conducive to recovery. Our facility is located in Garfield, New Jersey, a vibrant community that supports our mission of patient-centered care. You will work Monday to Friday, with a schedule from 9 AM to 5 PM, allowing for a structured work-life balance.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations to determine the functional abilities and limitations of your clients.
• Design and implement individualized treatment plans, adjusting them as necessary based on client feedback and progress.
• Deliver one-on-one therapy sessions focused on improving mobility, enhancing strength, restoring balance, and alleviating pain.
• Utilize evidence-based practices and innovative rehabilitation techniques to achieve optimal patient outcomes.
• Monitor patient progress meticulously, documenting changes in condition and treatment efficacy in accordance with regulatory guidelines.
• Engage in collaborative care with nursing staff, caregivers, and fellow therapists to ensure a cohesive approach to patient management.
• Educate clients and their families on strategies for long-term health, including body mechanics, fall prevention techniques, and best practices for mobility.

Patient Population / Client Focus

Your primary focus will be on geriatric patients, specifically those seeking to improve their quality of life and regain independence. Most clients will be adults aged 40 to 65 years, often facing challenges related to aging, injury, or chronic conditions. Your compassionate approach will not only aid in their physical recovery but also support their emotional well-being during therapy.

Qualifications

• Current and valid New Jersey Physical Therapist (PT) License.
• Bachelor's Degree in Physical Therapy from an accredited program; Doctorate or Master's degree preferred.
• Demonstrated competency in geriatric physical therapy and knowledge of relevant regulations.
• Exceptional interpersonal and communication skills, facilitating effective interaction with clients, families, and a diverse healthcare team.
• Ability to work autonomously in home settings, adapting to unique client needs and environments.
• Strong commitment to patient-centered care with a compassionate approach.

Preferred Experience

• At least two years of professional experience working with geriatric populations in a rehabilitation or assisted living environment.
• Familiarity with telehealth practices and home care therapy protocols.

Compensation and Benefits
