Work Environment

This position is based onsite at an assisted living facility, which provides a nurturing environment for our patients. The work setting allows you to engage with community members directly, performing both clinical care and community outreach. The role involves working typical hours from 9:00 a.m. to 6:00 p.m., with occasional evenings and weekend events for health screenings or community outreach initiatives, ensuring a total weekly workload of approximately 40 hours.

Key Responsibilities

• Perform comprehensive health assessments and screenings tailored to the unique needs of individuals and families within the community.
• Diagnose, treat, and manage both acute and chronic medical conditions, focusing on preventive care and health promotion strategies.
• Collaborate with local organizations and partners to create and implement effective health education programs that address community-specific health challenges.
• Ensure culturally sensitive care delivery while advocating for patients’ rights and their access to necessary healthcare services.
• Manage coordination of referrals and follow-up care, ensuring seamless continuity of care for all patients.
• Maintain accurate and up-to-date medical records, complying with all regulatory standards and best practices.
• Engage in community outreach activities, including health fairs and local events, to promote health education and wellness screenings.
• Continuously monitor and stay informed about trends in community health, utilizing evidence-based practices to enhance patient care.

Patient Population / Client Focus

This role primarily focuses on vulnerable populations residing in the assisted living facility, including older adults and individuals with chronic health conditions. By providing tailored health assessments and interventions, you will help address their specific healthcare needs, ensuring they receive appropriate care and support.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner.
• Active nursing license in the state of Maryland, compliant with all local regulatory requirements.
• A minimum of 3-5 years of clinical experience, ideally within community health or public health settings.
• Demonstrated strong interpersonal and communication skills, with the ability to connect effectively with diverse populations.
• A demonstrated commitment to social justice and health equity, with a focus on advocating for vulnerable communities.

Preferred Experience

• Experience working in an assisted living or similar setting, understanding the unique needs of older adults.
• Familiarity with local community health resources and services designed for populations facing health disparities.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 140,000 per year, which will be commensurate with your experience and qualifications.
