Key Responsibilities\n

• Establish strategic partnerships with community services to enhance aftercare arrangements for patients and their families, ensuring a seamless transition from clinical support to real-world applications.

• Utilize effective conflict resolution techniques to address and manage client resistance, fostering an environment of trust and engagement with individuals, families, health team members, and community stakeholders.

• Maintain accurate patient records by documenting assessments, treatment plans, interventions, interactions with relevant agencies, and case closures, adhering to the highest standards of social work documentation.

• Interpret and apply the latest DSM criteria in assessments of child and adolescent disorders, demonstrating a thorough understanding of developmental milestones and age-appropriate interventions.

• Utilize behavior modification principles, assertiveness training, and supportive therapy techniques to nurture positive client outcomes.

• Conduct comprehensive assessments to evaluate patients and their families’ medical, social, and emotional needs through effective interviewing and consultation.

• Periodically review and revise treatment plans based on ongoing psychosocial assessments, focusing on client strengths, concerns, cultural values, and spiritual beliefs.

• Facilitate educational and therapeutic group sessions, recording patient goals, progress, and responses to group activities to track therapeutic effectiveness.

• Participate in treatment-planning meetings, collaborating with interdisciplinary teams to discuss patient progress, strategies, and necessary updates to treatment plans.
