Key Responsibilities\n

• Conduct thorough clinical assessments to better understand patient needs and circumstances.

• Maintain accurate and timely clinical documentation using our electronic medical record (EMR) system.

• Provide ongoing education and emotional support to patients and their families, equipping them with resources to navigate their journey.

Patient Population / Client Focus\n

In this role, you will primarily work with terminally ill patients and their families, nurturing an understanding of their emotional and practical needs during a challenging time. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
