Work Environment

The position is based onsite in an outpatient office setting within our Skilled Nursing Facility. Here, you will engage directly with residents, healthcare staff, and their families in a vibrant and collaborative atmosphere. Operating Monday to Friday from 9 AM to 5 PM, your work will play a critical role in delivering quality care and rehabilitation.

Key Responsibilities

• Conduct comprehensive evaluations to assess each resident's physical capabilities, including mobility, strength, and functional performance.
• Design and implement tailored treatment plans focusing on pain relief, increased mobility, and fall prevention strategies.
• Administer a variety of therapeutic interventions such as manual therapy techniques, customized exercise regimens, and balance training exercises.
• Incorporate appropriate assistive devices and mobility aids into treatment plans to enhance patient autonomy and confidence.
• Collaborate with a multidisciplinary team, including nursing staff and occupational therapists, to ensure cohesive and holistic care delivery.
• Provide education and resources to residents and their families about home exercise programs and safety precautions to maintain progress outside of therapy sessions.
• Keep accurate documentation of patient progress and treatment outcomes, adhering to SNF regulations and best practices.

Patient Population / Client Focus

The primary focus of this role will be on elderly residents recovering from surgeries, injuries, or chronic conditions related to age. You will work with individuals who may be dealing with various disabilities or impairments, necessitating a compassionate, tailored approach to care that supports both physical rehabilitation and emotional well-being.

Qualifications

• Must possess an active and valid Physical Therapy license in the state of Virginia.
• Doctor of Physical Therapy (DPT) or a Bachelor's/Master's degree in Physical Therapy from an accredited institution is required.

Preferred Experience

• A minimum of 2 years of professional experience in a Skilled Nursing Facility or similar geriatric rehabilitation setting is highly preferred.
• Familiarity with long-term care regulations and a strong understanding of the rehabilitative needs of older adults.
