Key Responsibilities\n

• Build trusting relationships by establishing rapport quickly with patients and their families, utilizing active listening and clear communication free of jargon.

• Function as a collaborative member of a multidisciplinary team, engaging with behavioral health providers, primary care professionals, and patients to deliver cohesive care.

• Conduct evidence-based screenings and assessments for various mental health conditions, including depression, anxiety, and substance use disorders, and facilitate referrals as necessary.

• Create and manage a patient caseload, providing follow-up interventions tailored to individual needs.

• Utilize electronic medical record systems to document patient encounters accurately within 48 hours, ensuring continuity of care.

• Develop and implement integrated care plans, coordinating services among patients, family members, and treatment teams to address comprehensive health needs.

• Safeguard patient confidentiality in all aspects of communication and documentation.
\n
• Participate actively in supervision, case conferences, quality improvement initiatives, and team meetings to enhance service delivery and professional growth.

• Respond promptly to phone inquiries from patients, families, and other providers regarding appointments and concerns.

• Identify community resources and support services that align with patient goals, fostering connections to enhance patient wellbeing.

Qualifications\n
