Key Responsibilities\n

• Engage with patients and their family members to establish rapport quickly, utilizing active listening and non-judgmental communication techniques.

• Collaborate effectively with an interdisciplinary team, including behavioral health and primary care providers, ensuring integrated care delivery.

• Conduct both brief and comprehensive, evidence-based screenings for mental health conditions such as depression, anxiety, and substance use disorders.

• Develop and maintain a patient caseload, providing follow-up interventions to support ongoing mental health needs.

• Document all patient interactions and updates in the electronic medical record (EMR) system within 48 hours to enhance care coordination.

• Create and implement tailored integrated care plans that facilitate crucial linkages to necessary services within the community.

• Prioritize patient privacy and confidentiality in all communications and documentation efforts.
\n
• Participate in supervision, case conferences, and departmental meetings to contribute to team-based service delivery and quality improvement initiatives.

• Facilitate referrals to community resources, supporting the holistic needs of patients by connecting them with relevant services.

• Fulfill additional responsibilities as required to meet the dynamic needs of the organization.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will primarily interact with students and their families within school settings, focusing on increasing access to mental health resources for underserved demographic groups. This role emphasizes cultural competence, providing care that recognizes and respects the unique backgrounds and challenges faced by our patient population.

Qualifications\n

• Master’s degree in Social Work or a related Behavioral Health field.

• Active Licensed Clinical Social Worker (LCSW) certification is required.

• At least 3 years of professional experience in the Behavioral Health field, with a focus on mental health in a primary care or school-based setting.

• Competency in trauma-informed care practices and a foundational understanding of social justice frameworks as they relate to service delivery.

• Knowledge of best practices in behavioral medicine and evidenced-based interventions for mental health conditions.

• Proficiency with electronic medical records systems, data charting, and basic office software.

• Bilingual proficiency in English and Spanish is preferred.

Compensation and Benefits\n

The annual salary for this position ranges from USD 90,000 to 100,000, commensurate with experience and qualifications.
