Work Environment

This position is located onsite at our state-of-the-art rehabilitation facility in Punta Gorda. Key Responsibilities

• Conduct thorough evaluations to assess the functional ability of patients by applying a range of diagnostic, prognostic, and functional tests related to muscles, nerves, joints, and overall physical performance.
• Develop and implement individualized treatment plans based on evaluations and in collaboration with the interdisciplinary team, ensuring alignment with physician orders.
• Deliver comprehensive physical therapy services, including manual and therapeutic exercise, proprioceptive training, and functional mobility enhancement.
• Supervise and mentor physical therapy assistants in delivering therapeutic services, ensuring adherence to established care plans and state regulations.
• Monitor patient progress, observing, recording, and reporting any changes in conditions, adjusting treatment plans as necessary to optimize patient outcomes.
• Evaluate and make recommendations regarding home modifications and assistive devices to enhance patient independence during home evaluations.
• Maintain accurate and timely clinical documentation, including evaluations, progress notes, and discharge summaries, compliant with regulatory standards.
• Participate in interdisciplinary team meetings to discuss patient progress, revise care plans, and ensure cohesive treatment strategies.
• Educate patients and their families about treatment goals, therapeutic exercises, and device usage to empower them in their recovery journey.
• Complete additional administrative duties as assigned to support the overall goals of the facility and the organization.

Patient Population / Client Focus

The ideal candidate will work with a varied patient population, including individuals recovering from orthopedic surgeries, neurological conditions, and those in need of rehabilitation due to injuries or chronic pain. Our clients range from young adults to elderly patients requiring different treatment modalities tailored to their specific needs and recovery goals.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• A minimum of one (1) year of clinical experience in a physical therapy setting, preferably within a rehabilitation facility.
• Current state licensure in good standing and CPR certification.
• Demonstrated commitment to providing exceptional patient care and a team-oriented approach to rehabilitation.
• Strong communications skills with an emphasis on patient and family engagement.
• A proactive approach to professional development and a desire to stay current with advancements in physical therapy practice.

Preferred Experience

• Experience working in an outpatient rehabilitation setting.
• Background in managing complex patient cases and leading interdisciplinary care initiatives.
• Familiarity with electronic medical record (EMR) systems to maintain comprehensive patient documentation.
• Advanced training or certifications in specialized therapy techniques, such as dry needling, manual therapy, or sports therapy.

Compensation and Benefits
