As the Admissions Director , you will play a key role in managing the facility's admissions process while helping maintain census growth and providing outstanding service to residents, families, and referral partners.
Responsibilities include:
Coordinate and manage all admissions, discharges, and inquiries
Oversee the complete admissions process, ensuring all required documentation is accurate and completed on time
Evaluate referrals and assist with admission decisions
Maintain accurate census reporting and resident tracking
Collaborate with nursing, the business office, and interdisciplinary teams to coordinate admissions and bed availability
Build and maintain strong relationships with hospitals, case managers, and referral sources
Monitor referral activity, admissions trends, and discharge planning
Prepare and provide weekly census and admissions reports
Deliver exceptional customer service to residents, families, and referral partners
Support census development through proactive outreach and relationship building
High school diploma or equivalent required
Admissions experience in a skilled nursing facility (SNF) or long-term care (LTC) setting preferred
Excellent communication and relationship-building skills
Strong organizational and multitasking abilities
Positive, energetic, and professional attitude
Ability to work independently and collaboratively
Basic computer proficiency
Knowledge of healthcare admissions processes is a plus
